To register for the Supervisory Officer's Qualification Program, please complete and submit the SOQP Application Package to our office through email at SOQP@lakeheadu.ca

NOTE: While Modules can be taken in any order, candidates must successfully pass each module to continue. If a candidate does not complete a module in a satisfactory manner, they must re-enroll and successfully complete said module before continuing in the program.

Business Stream Applicants

Applicants registering for the Supervisory Officer's Qualification Program via the business stream should use the following application form: SOQP Business Application.

Please Note: Business stream applicants must show proof of having completed courses in School Board Administration and School Board Finance, OR have the following attestation form completed by their director or superintendent stating that they have equivalent education and experience.

Supplemental Forms

Upon successfully registering for the Supervisory Officer's Qualification Program, applicants will be required to submit the following forms:

• Leadership Profile - This profile is to be submitted on the first day of the Module to your instructor on Desire2Learn

• Practicum Proposal Form - This form needs to be submitted before your start your practicum. Only one form needs to be submitted for all modules of the program.
