Once you have read the above policy, please complete the pledge form (link below) and return it to your TA or lab instructor during the first week of lab (when you do check-in).  Only once you have submitted this form will you be eligible to purchase a breakage card (available for purchase in the lab; required prior to performing first experiment).  The completed pledge form will be carried forward each year you are enrolled in a chemistry course:
